Medical Forms

 
If your child/camper has any of the following medical needs, please follow the directions below regarding required medical forms.*

 -  If your child has an EpiPen for a FOOD ALLERGY &/or ANAPHYLAXIS EMERGENCY, please fill our the following forms:
   * Camper Medication Form
   
 - If your child has food allergies / intolerance that does NOT require an EpiPen, please fill out the following form:

 - If your child has asthma, please fill out the following forms: 
 - If your child will be bringing any medication to camp INCLUDING OTC (over-the-counter) medications, please fill out the following form:

Please complete and submit the appropriate forms no later than two weeks before their first day of camp. If you have any questions or concerns regarding your child's medical care, feel free to reach our to Camp Directors. 
 
*2018-2019 Levine Academy students that have previously submitted these forms for the prior school year, are not required to resubmit camp form (unless there are changes). Levine Academy students for 2019-2020, will need to fill out the Back-To-School packet at a later date.

FAQs

DO YOU HAVE TO BE ENROLLED AS A LEVINE ACADEMY STUDENT TO ENROLL IN A SUMMER PROGRAM?

Not at all! Although many of our summer program attendees come from Levine Academy families, all students are welcome. 

WILL LUNCH BE PROVIDED DURING FULL-DAY CAMPS?

No, students will need to bring a bag lunch for full-day camps. 

When packing lunches, please remember Levine Academy is a kosher facility and follows Kashrut (Jewish Dietary Laws). 
- No Meat (Only pareve, dairy or vegetarian food is permitted. Cold foods are the best. Fish with fins and scales such as Tuna, Tilapia and Salmon ARE considered pareve.)
- Sharing? Remember that food for birthday parties or to be shared at school must be certified Kosher. 
Foods that do not need a Heksher (Kosher symbol) are Milk, Fruit Juices, Fruits and Vegetables.
