To complete the Application for Admissions, you will access the Admissions portal. Your login information, including instructions to create a password, is sent to the email you use when you inquire or start an application.

• Transcript/School Records Release: Submit a Transcript Release Form to your child’s current school. Records should be sent directly to Levine Academy at admissions@levineacademy.org.
• Authorization for Exchange of Confidential Information: Complete this form to include your child’s current teachers, school, and any specialists they are or have been working with.
• Developmental Profile: Provide detailed insights into your child’s development, personality, and any relevant background information.
• Teacher Recommendation: If applicable, provide the teacher’s name and email to request a recommendation.
• Immunization Records: Submit a current immunization record.

Once all supplemental documentation has been received, we will schedule a full-day visit for applicants entering Kindergarten through 8th grade. During this visit, our Learning Support team will conduct a brief evaluation, focusing primarily on math, reading, and writing skills.

All screenings and evaluations must be done in person on our campus.

The Admission Committee will carefully review each completed application, including visit day evaluations and supplemental materials. Admissions decisions, along with any tuition assistance information, will be communicated to families via email.
